RN Care Management™
Catalina's innovative Care Management service provides
Nursing Coordination of Care for seniors who do not need help in their
homes at this time, who have private or family caregivers, or who may
not have family present to coordinate care.
As we all have experienced, our health care system is complex, fragmented,
and difficult to navigate. Elders and those with chronic illnesses or
disabilities are the least likely to obtain the resources and care they
need to maintain independence, and to prevent hospitalization and premature
nursing home placement.
This adult care service assigns a RN Care Manager who is there as your
advocate, educator, and consultant to assure your maximum possible health,
independence, and peace of mind.
RN Care Management is there as a safety net to assure:

Medications and treatments are understood and followed

Community services are provided and coordinated

A RN is available 24 hours a day to evaluate changes in health
status, to communicate with the physician and family, and to take
emergency action when needed.

1. What is "RN Care Management?"
RN Care Management is Catalina's professional Nursing evaluation
and "total" management of care for your optimum health. RN Care
Management is a collaborative process that evaluates, plans, coordinates,
monitors, and supervises your health care options and other senior
care needs. RN Care Management assures that the living environment
is appropriate and safe, medications and treatments are understood
and followed, that community services are coordinated and provided,
and that there is an RN available 24 hours a day to evaluate and
address changes in health status.
2. Don't social workers do this?
Social workers do "case management" which focuses on long-term social
and financial planning. RN Care Management provides the information,
resources, and direction needed to create a quality plan of senior
care addressing your total health and wellness. This "total" plan
of care monitors your health care concerns, as well as provides
proactive referral solutions for your emotional and social needs.
Our RN Care Managers are able to arrange for all the adult care
support services necessary to provide total care and absolute peace
of mind.
3. What is "coordination of community services?"
There are many senior services available in the Tucson community
which can support your goal of health, wellness, and independence,
but of which you may not be aware. Your RN Care Manager will recommend
the best Tucson community resources for you, and arrange for you
to receive them, without your having to bother with administrative
details.
4. I'm not ill. How will RN Care Management benefit me?
Your RN Care Manager, a licensed Registered Nurse, is experienced
in assisting healthy seniors to recognize and manage changes in
health status, which may require a doctor's care. We also connect
you to the best Tucson community resources to help you realize your
goal of aging with dignity. At Catalina, we are experts in assisting
people to age with independence and safety despite frail health
or the aging process. RN Care Management is an additional level
of security for you.
5. I have private help now. Do I need Care Management?
Yes. We can assist you in preparing a care plan to be followed by
your private employee; we can monitor your health and assure the
correct use of your medications and treatments; we can refer you
to other Tucson community resources. We have an RN on-call 24 hours
a day for any urgent health concerns you or your caregiver may have.
Your RN Care Manager can assure you and your family that your interests
are being served.
6. My family takes care of me. Would RN Care Management help
them?
Yes. Family Caregivers can feel more confident in caring for you
with an RN Care Manager to coach them. Our expert RN Care Managers
will work with you to tailor a plan of adult care to accommodate
your family's individual situation. Our goal is to maintain the
well-being, independence, and dignity of the individual while remaining
sensitive to the family's needs and resources.
